SIM swap for lost phone?

Lost a phone that we recently activated a 1GB/12-month prepaid FP SIM in. I have a few unactivated FP SIMs for "free" accounts lying around, and would like to move my 12-month account to one to use it another phone.
How do we do a SIM swap?
