Having problems porting out to Mint Mobile - inconsistent information

I got following message on the transfer web page of Mnt Moble:
"The information provided is not consistent with your previous carriers records. Please check with them and try again."
I've opened a request to confirm the information I got is same to the one on Port-Out page. Would you please help to resolve this issue?
Also, I am not sure if the value of "Phone Number" should start wtih area code instead of "1", the nation code? That is, my phone number valude has 11 numbers instead of 10. Thank you very much!
