PUK code request

I recently put my SIM (which I haven't used in ages) in a new phone, and when messing around with the settings, I selected SIM lock, not realizing that I didn't know the PIN for the SIM. The phone is now locked when the SIM is installed, and requests a PUK code, which I do not have.
