Incorrect billing - Being billed without authorization

I haven't used the account for years and the phone number is already gone. I just noticed my credit card was charged $4.03 for the past 4 months from Freedom Pop! I never authorize these payments or accepted these billing amount. I would like to request a full refund of 4x$4.03 = $16.12 to my credit card. Furthermore, I want to close my account permanently and please do what ever you need to NOT BILL ME ANYMORE! Thank you!
