Phone Number Disappeared and Phone Deactivated

Hi,

I have been using this phone and number for the past two years and now it suddenly asks me to activate my phone. When I login to my account it shows no number there and asks me to sign up and activate my phone. That is wrong. Could you please help to check ASAP? Thanks.
