Can't send or receive calls or texts, data doesn't work at all

I can see that I was able to receive a text as recently as July 5th, and a call as recently as June 29th, but when I needed data and call/text functionality on the 12th on a road trip, I was unable to do any of that, and since then there have been times when I needed a text for two factor that I wasn't able to do, so this is causing serious problems. On my phone, when I'm away from wifi, I see a notification that says I should "sign in to network" on my data connection, but flicking that just takes me to an error page. I don't know when the problem started, but it seems it was some time between the 5th and the 12th.

  Forgot to add, I've tried restarting my phone, switching to airplane mode for a while, double checking APNs, ejecting and reinserting my SIM, and probably a few other things to try to get service to come back and none of it has worked.
